Display Name = (Midwest SIP Account ID)
User Name = (Midwest SIP Account ID)
Password = (Midwest SIP Service password)
Authorization User Name = (Midwest SIP Account ID)
Domain = midwestsip.com
Domain Proxy = check register with domain and receive incoming calls
Select Proxy and the address = sip.midwestsip.com
Click OK and make calls
