Configure Polycom IP Phones to work with Midwest SIP account

Polycom IP 450 configuration - (may be the same for most Polycom IP configurations)

First, reset phone defaults.
boot phone, press menu, settings, advanced, type in password (456) then enter, admin settings,
reset to default, reset local, are you sure, yes. phone will then restart.
after reboot, press menu, settings, advanced, type in password (456) then enter, admin settings,
reset to default, reset device settings, are you sure, yes. phone will then restart.
Connect to phone using a web browser: http://_local ip) find in (menu key) (status)
(network) (tcp/ip)
Polycom phone Authentication window: type in Polycom for username and 456 for password.
